		<title>Joint Statement on Anaesthesia-Related Mitochondrial Pharmacogenetic Risk in Individuals with Venezuelan Maternal Ancestry</title>

					<description><![CDATA[<p>&#8211;European Society of Anaesthesiology and Intensive Care (ESAIC)European Society of Paediatric Anaesthesia (ESPA)European Reference Network (ERN) EURO-NMD Mitochondrial Working GroupEuropean Mitochondrial (E-MIT) Society Background Recent communications from major anaesthesiology societies have highlighted emerging reports of severe neurological complications, including basal ganglia infarction and death, following routine general anaesthesia in previously healthy individuals with Venezuelan maternal ancestry. Most reported cases involved exposure to sevoflurane, although details regarding duration and concentrations of exposure remain limited. Since word of these cases has spread, additional cases in Europe and the United States have been identified. Concurrently, on February 18th an Anaesthesiology publication summarizing a Report from the Chilean Ministry of Health has outlined a possible mitochondrial pharmacogenetic susceptibility to severe neurologic events after general anaesthesia(doi: 10.1097/ALN.0000000000005935). Experimental and mechanistic data published in Anesthesiology(doi.org/10.1097/ALN.0000000000006029) suggest that sevoflurane, at concentrations observed during clinical procedures, can inhibit mitochondrial respiratory complex I, resulting in a significant reduction in cellular oxygen consumption and energy production. Genetic testing was performed in ten of the fourteen initially reported cases, and all carried the mtDNA m.11232T&#62;C homoplasmic variant of mitochondrial DNA, affecting the NADH dehydrogenase 4 (MT-ND4)  gene, a component of complex I of the electron transport chain. Although the precise pathogenic mechanisms remain under investigation, the convergence of clinical observations and emerging molecular evidence has raised concern regarding a potential ancestry-related mitochondrial susceptibility to certain anaesthetic agents. Position Statement The European Society of Anaesthesiology and Intensive Care, the European Society of Paediatric Anaesthesia,the ERN EURO-NMD Mitochondrial Working Group, and the E-MIT Society jointly acknowledge the following: Nature of the Reported Risk Reported anaesthesia-related neurological complications appear to be associated with mitochondrial genetic susceptibility, particularly the homoplasmic mtDNA variant m.11232T&#62;C in the MT-ND4 gene. This variant has been reported almost exclusively in individuals with Venezuelan maternal ancestry. In affected individuals, clinical manifestations of mitochondrial dysfunction are absent prior to anaesthetic exposure. Current evidence remains limited, and further research is needed to clarify the magnitude of risk, the mechanisms involved, and the role of different anaesthetic agents. Identification of Individuals Potentially at Risk During pre-anaesthetic evaluation, clinicians may consider asking patients about maternal Venezuelan ancestry, as mitochondrial DNA is inherited exclusively through the maternal lineage. Patients with direct maternal lineage from Venezuela may be considered potentially at risk until further evidence clarifies the clinical relevance of this genetic variant. A detailed history should also include questions regarding: &#8211; Previous unexplained severe neurological events following anaesthesia&#8211; Family history of unexpected complications following anaesthesia&#8211; Unexplained postoperative neurological deterioration in otherwise healthy individuals Both children and adults may potentially be affected. Genetic Testing Mitochondrial DNA sequencing can identify the MT-ND4 m.11232T&#62;C variant. Important considerations include: &#8211; Some genetic laboratories may have historically classified this variant as a benign polymorphism.&#8211; Consultation with clinical geneticists or mitochondrial disease specialists is advisable when testing is possible or considered.&#8211; The availability and implementation of genetic screening may vary between countries and centres. At present, routine population-wide genetic screening cannot be universally recommended. However, testing may be considered in individuals with Venezuelan maternal ancestry undergoing planned surgical procedures when testing is accessible and feasible. Considerations for Anaesthetic Management In patients considered potentially at risk, and in the absence of definitive genetic results, anaesthetic management should be carefully considered. Possible risk-mitigation strategies may include: &#8211; Avoidance of Volatile Anaesthetic Agents. Because most reported cases involved exposure to sevoflurane, clinicians may consider avoiding volatile anaesthetic agents until further evidence becomes available.&#8211; Total Intravenous Anaesthesia (TIVA)as anaesthesia of choice. Propofol may be used cautiously, at low doses and for the shortest possible duration. One strategy could be to combine propofol with other adjuvants that decrease its consumption, such as dexmedetomidine. No experience has been reported so far with remimazolam in this context.&#8211; Whenever appropriate for the surgical procedure, regional anaesthesia techniques may be considered. Monitoring Enhanced intraoperative monitoring may include: &#8211; Processed EEG monitoring (pEEG) to assess depth of anaesthesia&#8211; Near-infrared spectroscopy (NIRS) to monitor cerebral oxygenation&#8211; Measurement of blood lactates level, as in other mitochondrial diseases, although no data are available so far on their usefulness in this pathology Conclusion This statement reflects the current understanding and expert consensus regarding a potential anaesthesia-related mitochondrial pharmacogenetic susceptibility in individuals with Venezuelan maternal ancestry. Given the limited evidence currently available, this document does not constitute a formal clinical guideline. Rather, it aims to increase awareness among anaesthesiologists and encourage cautious perioperative management while additional evidence is gathered. The participating societies will continue to monitor emerging data and will update recommendations as further evidence becomes available.</p>

The participation of the Mitocon, the Italian association of mitochondrial patients will add the patients’ perspective to the school.</p><p>The school will be held in person from <strong>January 18 to 22, 2026 at the Grüner Baum Hotel</strong> in Bressanone, Italy and will be open to 40 attendees from across Europe.Mitochondria are organelles often referred to as the energy plants of the cell as they are mainly responsible for converting energy derived from food into ATP, the high-energy molecule used by the biochemical systems of cells.</p><p>The series of reactions that lead to the synthesis of ATP forms the mitochondrial respiratory chain consisting of respiratory complexes I-IV and ATP synthase. A unique feature of mitochondria is that they have their own genetic material (mitochondrial DNA) that contains some essential genes of the respiratory complexes, although most of the genes that code for proteins found in mitochondria are found in nuclear DNA. The malfunction in the processes that lead to the synthesis of ATP due to mutations in the mitochondrial or nuclear DNA can give rise to mitochondrial diseases, rare genetic pathologies, characterized by extreme heterogeneity and clinical, biochemical and genetic complexity. The role of mitochondria, however, is not<br />limited to the synthesis of ATP, but includes a large number of essential metabolic pathways (e.g. the Krebs cycle, beta oxidation etc.), the regulation of cellular calcium levels, the synthesis of nucleotides etc&#8230;</p><p><br />The European Winter School in Mitochondrial Biomedicine responds to a need felt at an international level to improve the interaction between basic researchers and clinicians engaged in the study of mitochondrial biology and mitochondrial diseases.</p><p>The cross-contamination that the course aims to promote will train the next generation of researchers attentive to the problems of mitochondrial diseases and will increase clinicians&#8217; awareness of the potential and problems of basic research. The school will include the holding of seminar lessons and small thematic workshops ensuring close interaction between attendees and faculties. All the topics will be treated both from a clinical point of view and from that of basic biological mechanisms with the contribution of national and international experts</p></div></div></div></div>								</div>

                <h2 class="elementskit-faq-title">Tell us about your research background and goals.</h2>
            </div>
            <div class="elementskit-faq-body">
                During my undergraduate studies at the University of Cambridge, I benefited from being surrounded by a highly academic environment which piqued my interest in medical and biological research. Later, as a London paediatric trainee, I became very interested in metabolism and genetics. I then secured a post at Great Ormond Street Hospital where I decided to specialise in paediatric metabolic medicine and have since developed a special interest in mitochondrial disorders. My research interests include natural history studies, clinical guideline development, understanding pathophysiological mechanisms, development of preclinical models and gene therapy approaches for treating mitochondrial disorders. Recently I was appointed via UK-wide recruitment to a competitive academic clinical lectureship post at University College London which is a significant career milestone and has enabled me to undertake further research in parallel with my subspecialty training. My goal is to become a clinician scientist active in translational research geared at developing much-needed effective disease modifying therapies for primary mitochondrial disorders of diverse genetic causes.             </div>

                <h2 class="elementskit-faq-title">What is something you are proud you&#039;ve accomplished so far as a Researcher?</h2>
            </div>
            <div class="elementskit-faq-body">
                My work on gene therapy for primary mitochondrial disease is my biggest research achievement. In 2018, I secured a competitive Action Medical Research clinical research training fellowship which enabled me to undertake a funded PhD at University College London where I developed a novel in-vivo gene therapy approach to treat DGUOK deficiency. As a clinician-in-training who initially did not have a background in molecular biology or experience with characterising animal models at the start of this research, I am particularly proud that I was able to work past these limitations and additional hurdles along the way (including the COVID pandemic) to complete the intended work with much experimental success, and to go on to be awarded my PhD in 2023. Since then I have presented my results at two international conferences, have had a manuscript of my research findings accepted by Molecular Therapy Methods and Clinical development and secured a large follow-on research grant to refine the therapeutic approach further.             </div>

                My laptop (arguably the most valuable thing I own), yoga mat (I am a huge proponent of the multiple benefits of regular yoga practice) and a pair of anti-fog swimming goggles (I love swimming and do so 2-3 times per week but really cannot ostensibly manage to do so without goggles!)            </div>

                <h2 class="elementskit-faq-title">If you could travel anywhere, where would you go?</h2>
            </div>
            <div class="elementskit-faq-body">
                At the time of writing this I am en-route to French Polynesia – a place that I have always wanted to go ever since I undertook my undergraduate medical elective in the Solomon Islands. These atolls are some of the most difficult to reach (&gt;20 hours of flying from London), the reefs that surround them are teeming in marine wildlife but, like many small island nations worldwide, are slowly sinking into the ocean so we do not know for how much longer they will be around. In addition to this I am welcoming the opportunity to disconnect, introspect and recharge in warmer climes!            </div>
        </div>

                <h2 class="elementskit-faq-title">Where is your happy place?</h2>
            </div>
            <div class="elementskit-faq-body">
                The ocean floor. One of the pastimes that I enjoy is scuba diving. It offers me an escape from day-to-day inner city life and a much needed chance to commune with nature. I find staring at the infinite blue humbling, mesmerising and almost meditative. In addition to meeting some beautiful sea creatures such as sharks, manta rays and turtles, along the way I have also met many amazing fellow-divers who also share a love for the ocean and marine wildlife. I am now absolutely addicted and when I am not working, I am most likely to be planning my next diving adventure! If anyone shares the same passion – do get in touch!            </div>

                ‘You must be the change you want to see in the world’ – Mahatma Gandhi.
I like to be reminded of this quote from time to time since I see it as a call to reject reticence and to take action. It helps me redefine my sense of purpose and how I can maximise my contribution to those around me both personally and professionally.             </div>

									<p>The Freya Foundation was founded in 2014 by Kelly and Dave after their daughter Freya was diagnosed with PDH deficiency a month before her first birthday in 2012.  There was very little information about PDH at this time, and Kelly wanted to try and do something to help bring families together and try and get more information about PDH deficiency by improving the understanding of the condition and hope for a better future for those affected.</p><p>Freya celebrated her 13th Birthday in July; an age Kelly &amp; Dave were told that Freya would never reach.</p><p>We are currently the only known UK charity that has commissioned a research project into PDH deficiency, in 2023 we became affiliated with MetabERN who are the European Reference Network for Hereditary Metabolic Disorders and at our meet up last November we announced a groundbreaking gene therapy research project which is the first of its kind in the UK and Europe for PDH Deficiency. We work closely with doctors and dieticians around the both the U.K. and Europe and have completed the first UK Natural History study for PDH deficiency.  </p><p>Each year we hold a family meet up bringing together families from across the U.K. These meet ups have become a vital source of support and give families the opportunity to share advice stories and just be together with other families that are going through the same.  For more information about The Freya Foundation please visit <a href="https://www.thefreyafoundation.co.uk/" target="_blank" rel="noopener">www.thefreyafoundation.co.uk</a></p><div> </div>								</div>
